When you’re young, fat is evenly distributed in the face. An abundance of collagen gives your skin a plump, youthful look. Aging or weight fluctuations can cause changes to the shape of your face. The lower face may gain fat while skin that was once tight may become loose and sag. Patients can get more advice about treating excess skin around the jaw, neck, and chin, known as jowls, at Madison, WI’s Radiance Skin Therapy & Laser Center.

Skin can lose elasticity and begin to sag as you age, often leading to the formation of jowls. Levels of collagen and elastin decline, while the face also loses fat. Jowls often occur when the skin becomes less elastic and it is difficult for it to maintain its shape, so it is more vulnerable to gravity. Factors such as genetics, stress, and lifestyle choices may cause jowls to look more pronounced. Sagging skin around the cheeks and mouth may fall to the jawline area.
A loss of upper face fullness can often contribute to jowls. Facial volume decreases as you age, often causing a hollowing around the eye sockets, temples, and cheeks. It can make certain areas of the face such as the lower face look heavier as well. The decline in collagen production with age coincides with this loss of volume.
There are many risk factors for jowls such as a history of sunburn, excessive or chronic alcohol use, smoking, chronic or severe dehydration, chronic or severe stress, lack of exercise, a family history of jowls, and extreme or rapid weight loss.
Almost everyone eventually gets this type of sagging skin. Although you may not always be able to avoid the loose skin and fat that cause jowls, there are some lifestyle changes you can make to help prevent them. Wearing sunscreen and protective clothing during prolonged exposure to sunlight, using moisturizers, applying creams that are high in antioxidants and promote collagen production, exercising regularly, and not smoking can help.
Although jowls are harmless and do not require medical attention, some people may feel self-conscious about having them and their self-confidence may be affected. KYBELLA® can have a positive impact on jowl fat by removing the fat cells that contribute to jowls and sculpting the jawline. Dermal fillers are another option for improving the look of sagging skin around the jaw. Fillers can be injected to fill in areas where fat has been lost from the skin. Lifting the cheeks with fillers can also have a subtle lifting effect on the skin around the jawline and corners of the mouth, improving the appearance of loose skin.
BOTOX® is also highly effective for improving the appearance of the jawline and neck area. In what’s known as a Nefertiti Lift, botulinum toxin injections are used on the lower part of the face, jaw, and neck. This treatment helps to lift and smooth the general neck area, creating a more sculpted jawline area. Sofwave™,a non-invasive skin tightening technology, targets the dermis, delivering ultrasound energy to stimulate collagen and elastin. This has a subtle lifting and tightening effect on loose skin around the chin and neck area to reduce the appearance of jowls.
Dermal fillers are another highly effective treatment option for improving the look of sagging skin around the lower face area. Facial balancing with fillers involves restoring volume, contouring the face, and addressing asymmetry. It enhances facial symmetry and proportion, providing a more aesthetically pleasing appearance.
Fillers can be injected to fill in areas where fat has been lost from the skin and boost volume, providing a subtle lift to sagging skin on the lower face as well. For example, temple fillers can play an important role in facial balancing. Though they don’t directly target jowls, these fillers have a cascading effect, providing a subtle lifting effect that improves the look of the jawline. These fillers can give the whole face a slight lift. Lifting the cheeks with fillers can also have a subtle lifting effect on the skin around the jawline and corners of the mouth, improving the appearance of loose skin. Jawline fillers also give the lower face a more defined appearance.
